MEDICAL RECORDS are available for up to 6 years after your last visit here. For patients under age 15, records will be kept until you turn 21.
If your new provider is affiliated with ACP (Arizona Community Physicians), AND I have seen you since August 1, 2013 (when I joined ACP), then your record is already in their computer system and you will NOT need to ask for records to be sent.
If your new doctor is not affiliated with Arizona Community Physicians, OR I have not seen you since August 1, 2013, then you will need to fill out a medical records release form. To get this, please to to website AZACP.com, click on FORMS, then click on MEDICAL RECORDS FORM. Or, you can get a medical records release form from your new doctor. Once this form is filled out with your new primary care provider's information and you have signed it, please fax it to ACP at fax 520-547-5834.
Please note ACP owns all records beginning 8/1/13. I still own records created between 4/1/10 and 7/31/13, but if I saw you after 8/1/13 your records were already copied into the ACP system.
My office is closed but the answering machine and fax still work in case you need to contact me. (Please fax medical records requests to ACP fax # 520-547-5834, not to me).
I cannot prescribe medications to former patients.
